To begin the enrollment process, just follow these steps:
1. Give us a call between 9 am and 3 pm (or leave a message after hours). We'd like to learn a little about you and your family's situation. After we know more about your family, we can advise the best course of action.
2. Schedule a time to come in for a tour! Come see a facility and meet the people who would potentially work with your family.
If at any point you have any questions, please don't hesitate to call us!
3. Complete and return the intake packet. You can choose from one of three options.
Option 1: Click here to download the form. You can fill it out on your computer and email it back to us. You can also print the form and mail, fax, or drop it off when once completed.
Option 2: Reach out to info@bc-talk.com or call 508-807-4996 and ask us to mail you a physical copy.
Option 3: Come by one of our clinics and we can hand you a physical copy.
